Instructions
    1. Before printing, remove your Web Browser PAGE HEADER and PAGE FOOTER (Select File, then Page Setup)
    2. Set your Browser Margins to  .25   (top,bottom,left,right) before printing.
    3. To select a Form, click inside the circle that precedes the form's name. 
    4. Enter a Booking Number , Select a Charge Group, Then Click on Create Form
    5. To print a Blank Form, Click on the Blank Form link located next to Document name
